Incident

An event where an AI system behaves in an unintended, harmful, or policy-violating way, whether discovered internally or reported by users. Incidents range in severity from minor output quality issues to serious safety failures, and how an organization responds, including documentation, root cause analysis, and remediation, is a key indicator of its AI governance maturity.
